TriHealth, Inc.'s affiliated hospitals and physician practice groups (collectively, "TriHealth") are pleased to offer our patients on line computer access to portions of their medical record through TriHealth's MyChart. If you believe that you would benefit from this service, you may sign up for access by completing a MyChart Patient Sign-Up Form and agreeing to these terms and conditions of use.


At this time, we anticipate that MyChart will provide the following services:

  • Ability to request an appointment, and review past and current appointments.
  • Ability to view selected portions of your medical record such as allergies, medications, histories, problem list and recent visit information.
  • Ability to receive selected lab results.
  • Communicate with your physician via secure messaging regarding health concerns.
  • Request prescription refills.
  • Links to other sites that will provide you health information.
  • Provide access to another adult's or your minor child's MyChart record.

Other services may be added as TriHealth implements MyChart.


I understand that MyChart contains selected, limited medical information from a patient's medical record and that MyChart does not reflect the complete contents of the medical record.

I understand and acknowledge that MyChart may contain medical information related to the treatment for physical and mental illness and other health information of a sensitive nature. I expressly authorize TriHealth to include mental health information in my MyChart and understand and agree that all proxies I designate will have access to such information.

I understand that MyChart should NEVER be used for urgent health care matters. The anticipated turnaround time for a response to a secure message is three business days. Therefore, if I believe I am experiencing an urgent or life threatening health condition I should phone my physician, go directly to the Emergency Department of a local hospital, or call 911.

I understand that my use of MyChart is not a substitute for traditional medical advice, consultation, diagnosis or treatment.

If your physician is out of the office or unavailable to respond to your secure email message, your message may be routed to another TriHealth provider.


I understand that MyChart is intended as a secure online source of confidential medical information. If I share my MyChart ID and password with another person, that person may be able to view my or the patient's health information, and health information about someone who has authorized me as a MyChart proxy.

I understand that my activities within MyChart may be tracked electronically and that entries I make may become part of the medical record.

I understand that access to MyChart is provided by TriHealth as a convenience to its patients and that TriHealth has the right to end access to MyChart at any time, for any reason, including any misuse of MyChart by me (or my proxy).

I understand that access to MyChart does not grant me or my proxy any right, license or interest in TriHealth's proprietary or confidential information or property and that I am not permitted to use, reproduce or distribute (e.g. cut and paste) any intellectual property of TriHealth contained on its website or in my MyChart medical record. For example, I understand that I am not permitted to copy any of the pages/screens of MyChart and paste them elsewhere as this would be an inappropriate use of TriHealth's proprietary information.

I understand that my use of MyChart is voluntary and I am not required to use MyChart or to authorize a MyChart proxy.

If I designate a proxy, I understand that it is my responsibility to contact my primary TriHealth location and tell them that I am revoking the proxy status when I no longer want the person I designated to have access to my MyChart information.

I understand that TriHealth providers may send me messages via MyChart. It is my responsibility to monitor these messages and respond in an appropriate manner. I must provide TriHealth with a valid email address and update TriHealth if I change my email account. TriHealth may send me emails via my personal email address to notify me that I have new MyChart messages. Messages sent via my personal email will not contain personal health information; however, I understand that any person with access to my personal email account will be able to see these messages.

I agree that all messages that are sent through MyChart will be regarding MY health care and conditions. I understand that asking for advice for a friend or family member is not allowed. Since these messages may become a part of my permanent medical record and will be perceived as my health information, it is important that no one else's information be reflected in my messages. TriHealth takes no responsibility for erroneous medical information that you provide via MyChart.

I understand that I may not transmit any information or message that contains unlawful, threatening, fraudulent, libelous, defamatory, obscene or abusive information or language. MyChart may not be used for any illegal or unethical activities. Hacking, attempted access or other forms of inappropriate access or attempted access to unauthorized areas or information on this site will lead to termination of use and potential legal prosecution.


Use of MyChart will be governed by Ohio law.


Both Federal and Ohio laws govern the confidentiality of medical information. TriHealth is dedicated to keeping your medical information confidential. Our employees are subject to strict standards on maintaining your confidentiality. By allowing you access to your medical information you must share this responsibility. Please read TriHealth's Notice of Privacy Practices for more information.


I understand that I will be given a unique access code by TriHealth to allow me access to MyChart. The first time I log into MyChart I will be required to create a unique user ID and password. I agree to keep my user ID and password confidential. Anyone with access to my user ID and password would have access to my personal health information, ability to add to my personal health information and message my providers as if they are me. It is my responsibility to change my password if I believe it has been compromised or shared with someone else. It is recommended that you change your password every 180 days. Every time I log into MyChart, my user ID and password will be logged in an audit trail in the computer system.

I understand that if TriHealth discovers that I have inappropriately shared my password or that I have abused my access privileges, TriHealth may terminate my participation in MyChart. I understand that TriHealth takes no responsibility for and disclaims any and all liability for damages arising from a breach of medical record confidentiality resulting from my sharing or losing my password.


I understand that MyChart may not always be available to me due to system maintenance, back-ups or failures. Access is provided on an "as is available" basis and I am not guaranteed access at any particular time.


All communications between you and TriHealth using MyChart are carried over a secure, encrypted connection.


I understand that TriHealth may provide links or reference to other sites but TriHealth has no responsibility for the content of such other site and shall not be liable for any damages or injury arising from that content. Any links to other sites are provided as merely a convenience to the users of this web portal.
